                 How do I access the online questionnaire?

                          The project coordinator will send you the link.

 

             I lost the link, what do I do?

                          Contact us and we will send you another link.

 

             Do I have to complete the questionnaire in one sitting?

                          Yes, you have to complete the questionnaire in one sitting.  It should

                          take you no longer than 40 minutes.  You cannot exit the questionnaire

                          and return.

 

             My computer crashed while I was completing the questionnaire.  What              do I do?

                          Contact us and we will send you a new link.

 

             My friend wants to be in the study, can they volunteer?

                          No, this study is a continuation of the study that you participated in                           previously.  You were selected for this study.

 

             I lost my consent form, can I get another one?

                          Contact us and we will send you another.

 

             When will I get paid?

                          You will receive payment within 2 weeks of completing the

                          questionnaire.  If you have not received your payment with 2 weeks,

                          please contact us.
